Clear Choice Health Plans
Clear Choice is a Bend, Oregon-based company. They serve Central Oregon,
Eastern Oregon, and the Columbia Gorge. Clear Choice Health Plans, Inc. was
formerly known as Central Oregon Independent Health Services, Inc. and
changed its name in May 2007. The company was founded in 1995 and is based
in Bend, Oregon.
Clear Choice offers three types of plans: Preferred Provider Organization
(PPO) Plans, Point of Service (POS) plans, and Health Savings Account (HSA)
qualified High Deductible Health Plans (HDHP).

$10,000 Deductible, $40 PCP, 30% In / 50%
Out-of-Network Non-PCP and Coinsurance

$5,000 Deductible, $30 PCP, $40 In / 50% Out-of-Network Non-PCP, Covered
